APPLICANT AGREEMENT In addition to a performance fee and depending on the type of event, additional services may be required. (e.g. piano, microphones, audio, stage platform, dance floor, lodging.) Please note: Transportation and food will be a requirement for most request. Any additional cost for such services will be the responsibility of the requesting organization. Additionally, completion of this form does not guarantee or confirm your request.
